Sternal Puncture Trainer

The sternal puncture trainer simulates the upper torso and neck. It is designed for practicing the collection of cytological material from the manubrium of the sternum and features anatomical landmarks such as the jugular notch, sternum and neck. The sternum model is filled with artificial blood, which can be aspirated while practicing the procedure. The manipulation area is designed as an easily replaceable insert.

The trainer is intended to improve the training of students, residents and practicing physicians preparing for certification and accreditation.

Skills trained and stages of working with the trainer:

• Performing the complete procedure from start to finish (from preparing all required materials to waste disposal);
• Local anesthesia of the tissues (demonstration without injecting solution);
• Performing sternal puncture (in vivo aspiration biopsy of the bone marrow), proposed by M.A. Arinkin in 1928: a Kassirsky needle (a special needle with a guard) is used to puncture the outer plate of the sternum and withdraw a small amount of bone marrow for cytological analysis;
• Identifying the correct puncture point: along the midline of the sternum at the level of the 3rd–4th intercostal space, perpendicular to the sternum, until a distinct "give" is felt as the anterior plate of the sternum is punctured;
• After the needle is inserted, a syringe is attached to it and used to aspirate a small amount of bone marrow, up to 0.5 ml;
• After the needle is withdrawn, the puncture site is covered with a sterile dressing;
• If the trainee advances the needle deeper than intended and punctures the posterior plate of the sternum, simulated blood flows into a reservoir, signaling a complication: through-and-through puncture of the sternum and bleeding from the puncture site.
